Social Prescribing Link Worker
social prescribing sutton / age uk sutton (employer)

Job Description

Be part of the London Borough of Sutton’s new Social Prescribing programme for adults aged 18+.

We are looking for Social Prescribing Link Workers to work with adults in Sutton, supporting them to find lasting solutions to social issues and personal challenges.

Social Prescribing offers patients attending Primary Care (e.g their GP) about non health-related issues, personalised support to set goals, connect with local services, and improve their quality of life. You will be linking patients with relevant activities, services, and support, helping them access longer term assistance where needed, and enabling them to move forward

This service is based at Age UK Sutton as they are the lead contract holder, but is for adults of all ages, and involves extensive partnership work with the voluntary and community sector in Sutton.

You will have some experience in a relevant role, and understand the way health and social needs can interact.  Good communication, research, and record keeping skills are essential. You will have access to training, clinical supervision, and regular support meetings with your peers in the service including specialist nurses, GPs, and other Link Workers.

This is an exciting role working across the London Borough of Sutton, and an opportunity to help to shape a new service in its early stages.

For an informal discussion about the role prior to applying, please contact the recruiting manager, Hilary Dodd - details can be found in the job pack.

Please note that we are also recruiting for a Head of Social Prescribing role to lead the service.

Please note that CVs not accompanied by a supporting statement will not be considered.

The client requests no contact from agencies or media sales.
