Salary:        £21,189 (full-time-equivalent: £22,400)

About Social Prescribing in Crawley

Managed by Crawley Community and Voluntary Service, social prescribing in Crawley sets out to improve the health and wellbeing of local people by supporting them to connect with community support and activities. We employ Social Prescriber Link Workers who support people referred by their GP Practice or Adult Social Care to access services, groups and activities that meet their social, emotional and practical needs.

Following a successful pilot in 2016/17, the Crawley and Horsham and Mid-Sussex Clinical Commissioning Group commissioned Crawley Community and Voluntary Service to develop and expand ‘Prescription Plus’ across Crawley.  The Project has been extended since then and we are now planning towards October 2020 when the service will align with West Sussex.

‘Prescription Plus’ aims to:

  • Link clients with sources of social, practical and emotional support that can help improve their health and well-being and quality of life
  • Promote self-management through the use of person-centred methods and an empowering approach that involve people in decisions about their support
  • Provide a bridge between Primary Care and sources of health and well-being support offered by the Voluntary and Community Sector

We are recruiting Social Prescribers to join the team.  The post-holders will be partnered with four GP surgeries in Crawley.  They will be responsible for taking referrals, meeting and assessing patients, and supporting them to make positive changes to their lives by connecting them with community activities. You will be part of an expert team developing and expanding this project in Crawley. 

Due to the nature of this role, it is essential that the post-holder has a full, clean driving licence and access to a fully insured vehicle.
