Job Description

Age UK Sutton is seeking a Home From Hospital officer to join its highly effective commissioned service, assisting older people at the point of discharge from hospital.

Age UK Sutton’s (AUKS) pioneering Home From Hospital Service provides practical support on discharge from hospital.  HFH is a service funded by the NHS through the Sutton Clinical Commissioning Group. The service supports older people (65+) in Sutton in order to:

• Reduce number of readmissions to hospital

• Aid discharge

• Increase motivation, confidence and social integration after a hospital stay

• Reduce unnecessary hospital admissions

The recruited post holder must have the skills to work across the service and quickly establish credibility with health and social care practitioners, patients, and their families. The right candidate will have a background in health, social care or voluntary sector provision, excellent communication skills, personal resilience combined with a passion for working with people in later life to enable them to set and reach meaningful personal goals.

Our Home from Hospital Service is delivered by a team of staff and skilled volunteers, led by the Head of Health and Care and working closely with our specialist Health and Social Care team, Home Support social enterprise and wider staff and volunteer team.

Age UK Sutton works closely with partners including GPs, Pharmacists, Hospital Staff and Sutton Health and Care to ensure that we work collaboratively to deliver patient centred services.
