Job Description

The Dementia Peer Support officer is a new role at Age UK Sutton, working within the wider Integrated Health Services team.

Age UK Sutton have been commissioned by London Borough of Sutton to provide peer support for people affected by dementia, both those living with dementia and their carers.  Age UK Sutton are working in partnership with Sutton Carers Centre for the carers' element of this new commission. 

The Dementia Peer Support programme is a new commission to end June 2022 to provide opportunities for people living with dementia and carers to come together in an informal setting for facilitated information and peer support sessions.

The purpose of the service is to inform, educate, support and provide structured activities with identified outcomes for attendees in that group setting through Regular drop-in information sessions (a mix of face to face or online) and facilitated peer support group.

The purpose of this role is to co-ordinate, organise and facilitate the successful and safe provision of services delivered in a group setting and to provide peer support opportunities for older people living with a diagnosis of dementia and for their carers.

Part of this service delivery is also to understand and review the needs of people affected by dementia to frame future service models, post June 2022.

This is a fixed term role with potential for extension.
