Age UK Croydon exists to make life easier for all older people in the London Borough of Croydon. We provide a wide range of services and activities and aim to help older people live as independently as possible within the community.

We are looking to recruit a support worker as part of a new service in Croydon, known as Personal Independence Coordinators (PICs). The PICs work alongside health and care professionals, adopting a multidisciplinary approach to working with older people who have long term health and social care needs. Most importantly, they ensure that care planning is influenced by the wishes identified by individuals, so that independence can be either improved or maintained.

The Personal Independence Coordinator Support Worker will perform a key role within the team, providing time limited support to older people and supporting the case load of the Personal Independence Coordinator (PIC) team.
