Job Description

Supporting vulnerable single adults who have been homeless and who have support needs. The service provides individuals with accommodation and floating support service to ensure that they settle into the service and, sustain their licence. A Support Coordinator will be assigned to the individual who will then provide them with the support they need according to their assessment of needs, their risk assessment and their care/support plan. Working with them to self develop and progress, reaching the targets they have set so that they can move on to being fully independent in the community.

Specifications

You must:

  • Have at least a level 2 in Health and Social Care,
  • Have knowledge or experience of social care practices and regulations
  • Have knowledge and experience in supporting people in the community
  • Have excellent communication skills and be able to converse with service users in an appropriate manner. Have confidence in communicating with other community services, and or local authority departments who may be supporting the service user.
  • Have an empathetic and understanding nature is vital
  • naturally, have an excellent understanding of equality and diversity guidelines
  • Understand the importance of confidentiality and be able to apply this to your role
  • Able to follow good working practices and procedures.
  • Have knowledge and understanding of writing care plans and risk assessments
  • Be proficient in recording and reporting, keeping records, updating information on the current systems.
  • Be knowledgeable in using IT systems including word and excel and be willing to learn how to use other recording and monitoring systems used to ensure the efficiency of the company.

If successful you will be joining the Floating Support Team who work with vulnerable adults with an array of complex support needs such as mental health, substance and alcohol misuse and homelessness.

Full- time and Part time positions available in a fast-growing organisation available for an immediate start with a competitive salary.

Role:

  • You will provide support to vulnerable homeless individuals who have identified needs.
  • You will together with, the service user develop a comprehensive and personalised support plan that will highlight their needs and include an action plan to aid them in achieving goals that they have agreed.
  • You will be expected to highlight further needs if they arise whilst the service user is accommodated within the service.
  • You will ensure that a risk assessment has been completed and that you have the information necessary to support the individual appropriately
  • You will be able to induct them into the accommodation and the service by accompanying them to the allocated property and introducing them to other service users that they will be residing with. Giving them background knowledge of the area if necessary, pointing out health and safety procedures, including fire security.
  • You will be expected to conduct regular key work sessions with the service users allocated to you ensuring that you provide them with the opportunity to add to the agenda for the key work meeting.
  • You will be expected to keep legible and comprehensive key work notes and other reports of your contact with your service users.
  • You will be expected to file any such notes appropriately and accurately
  • You will be expected to keep service user information confidentially
  • You will ensure that the service user is engaging with relevant community services to aid in their recovery and progression
  • You will ensure that you keep in touch with community services that are working with your service user to keep informed of their progression and intervene if you are informed that they are not engaging.
  • You are responsible for ensuring that your service user is engaging and if there are any other measures to be implemented to keep them engaged
  • You are to ensure that they are aware of the conditions of their licence and the house rules and support them as much as practicable to refrain from breaching.
  • You will be reminding them of their obligation to pay service charge so that they do not go into arrears, and that it is an important element of learning budgeting skills, preparing them for independence.
  • You will be expected to give the service user advice on health and safety issues
  • You will be expected to advise your service user on life skills and other topics that may aid them in their recovery and move on.
  • You will be expected to facilitate monthly house meetings and at least quarterly workshops.
  • You will be expected to keep up to date records of all key work sessions, house meetings, and workshops
  • You will be working with the other teams to ensure the safeguarding of the service users; the Security and Welfare team particularly.
  • You will take instruction from your Manager on any other relevant matters that will ensure that the service provided is of a high standard and taking into account the dignity of privacy and respect of the client.

This is the salary paid after a successful review of the probationary period.

Qualifications and Skills

Previous care sector or social care experience would be beneficial.

You must be IT / Microsoft Outlook literate

You must hold a full UK drivers licence

You must be able to pass a CRB check
