Job Description

We are looking for people who are enthusiastic, great at building relationships, motivated, willing to get stuck in, committed and reliable. The role of a support worker is highly demanding and you will be working to address complex needs and challenging behaviour by providing assertive, consistent and enabling support to people with complex mental health problems, Autistic Spectrum Conditions, learning disabilities and substance misuse issues. We deliver our support and housing services 7 days a week, 24 hours a day.

Essential

• An NVQ Level 2 in Health & Social Care (or equivalent) and / or experience in a relevant field are essential.

• DBS Certificate registered on the update service

• Car driver and use of a car

• Understanding of professional boundaries

• Good IT skills using MS Word and Smart Phone Apps for rotas and electronic notes

• Flexible approach to working 

Cultural awareness and ability to meet the different cultural needs of a variety of tenant groups

SUMMARY OF DUTIES AND RESPONSIBILITIES:

Tenant-related support tasks:
• Every three months, to assess Tenants’ support needs in all aspects of their daily living skills and design support plans in conjunction with the tenant, their families and carers (where appropriate), lead colleagues, care managers and relevant professionals.
• To review and complete Support Plans, Risk Assessments, Health Action Plans, Hospital Passports and Epilepsy Passport where appropriate.
• Complete budgeting plans with Tenants and provide support with money management skills
• Support tenants with shopping for food/household goods in keeping with budgeting plans
• Support tenants with meal preparation, cooking, baking and promoting a healthy eating routine.
• Arranging/supporting/monitoring of health appointments and routine health checks and GP appointments including:
✓ Drug and alcohol services
✓ Clozaril clinic blood testing appointments and monitoring smoking and alcohol levels
✓ Depot clinic appointments
✓ Dental check ups
✓ Optical check ups
✓ Specialist Health checks – e.g. diabetic, epilepsy, dietician, smear tests, Mammograms, Blood pressure and ‘Flu jabs
• Review and update Tenant Personal Information Sheet and other tenant-related documentation.
• To administer medication in line with the organisation’s Medication Administration policy and procedure for the safe administration of medicines.
• Plan, arrange and encourage social activities by the Tenants.
• To supervise and encourage gardening duties and other neighbourly and community responsibilities.
• To provide personal care to tenants as necessary.

Tenancy-related support tasks
• To Support the Tenants in keeping their homes safe and secure and carry out:
• Monthly health and safety audits of properties of key tenants
• Fire risk assessments
• Setting up tenancy related bills
• Contacting and liaising with Utility companies
• Ensuring that tenancy related bills are paid on time
• Housing Benefit claims (BECs) • Dealing with queries from DWP
• Support the tenant with day to day cleaning in the home, including regular ‘deep cleans’
• Ensuring the correct household refuse and recycling bins are put out for collection on the relevant days.
▪ To complete all necessary records of inspections undertaken, repairs needed, identified risks etc.

Miscellaneous/Administrative tasks
• Ensure session sheets are complete and up to date.
• Ensure CareLineLive Care Management Software notes are completed following each support session
• Complete timely and comprehensive incident reports
• Undertake Money Log Audits
• Undertake Medication Administration Record sheet (MARs) audits
• Complete Medication Return sheets • Complete feedback questionnaires with tenants
• Undertake mandatory and good practice training
• Attend and participate in supervision
• To observe and promote Blue Pits Housing Action’s Health and Safety Policy.
• Attend and participate in training and staff meetings. To attend other meetings as required.
• To participate in training as required for personal and professional develop and ensure that Personal Training records are maintained.
• To liaise with care managers in a professional manner and attend multi-agency care planning meetings as required.
• To carry out any other duties and responsibilities or activities that may be reasonably required by the Leadership Team.
