Job Description

This will be an initial 12-month contract which may be extended depending on the project outcome and funding

37hrs a week.

Mary Frances Trust (MFT) is a charity organisation established in 1994 by people who used mental health services, carers and professionals to support people with any mental or emotional health problems.

MISSION

The purpose of Mary Frances Trust is to:

  • work with people who experience any kind of emotional or mental health distress until they feel confident to get on with their life again

  • provide leadership and co-ordination in development and delivery of community mental health services in Surrey

For the last few years we have been one of the leads for Surrey-wide Community Connections service and we currently provide our services in: Mole Valley, Epsom & Ewell, Banstead, Elmbridge and Spelthorne.

Recently, we have been working in partnership with Surrey and Borders Mental Health NHS Foundation Trust (SABP) to co-develop rapid field testing of a new integrated Mental Health Service with GP Practices within the National Association of Primary Care (NAPC) Primary Care Network (PCN) model.

GP Practices in three localities are working together to deliver population-based care across a group of practices in 3 localities: Guildford North, Banstead (Surrey Downs) and North West Surrey.  The priority for the service is the development of Primary Care Mental Health Services to meet the needs of 4 key cohorts of patients

  1. People who have mental health needs but who do not meet the access criteria for IAPT or secondary care
  2. People who are in recovery and stable in secondary care mental health services who would be cared for within Primary Care
  3. People who have been diagnosed with Dementia and require community and carer support
  4. People with Severe Mental Illness who require physical health checks and medication reviews.

This is an exciting opportunity to be involved in developing and shaping a new and innovative approach.

MFT, as a lead provider of Community Connection, are looking to recruit a Community Connections - Primary Care Link Worker for the Banstead Area as part of this initiative.
