Job Description

Hosted by South West Yorkshire Partnerships NHS Foundation Trust, Altogether Better works with citizens and services where change is needed to improve health.

We are seeking two talented individuals (2 x 30 hours per week) to lead on the local delivery of our ‘Community Centred Practice’ model in Wakefield and Calderdale, supporting GP Practices and citizens to work together to co-evolve ways to respond to the rising pressures experienced in primary care.  This involves looking at the system as a whole, the entire population and everyone involved in delivering care, tapping into the enthusiasm, assets, experience and generosity of citizens who join this work as volunteer ‘Practice Health Champions’.

There will be 2 key strands to the work:

  • Engaging and supporting citizens to work as volunteer Practice Health Champions in their GP practice
  • Building relationships with staff teams in primary care to support ‘community centred practice’ to develop and become embedded in the practice
