We are now recruiting for a Carer Support Worker (Mental Health) - New Forest - 27 hrs per week

Reporting to the Carer Support Team Leader and based at Anchor House, Totton you will ensure that carers from the New Forest area who provide regular and substantial care for a family member receive support including liaison and co-ordination of:

• Assessments of carer needs and reviews of those needs where long term care is provided
• Carer support plans which are co-produced and have clear agreed outcomes
• Links to and liaison with the Community Treatment Teams, where relevant, with particular reference to the carers needs
• Supporting existing carer groups and supporting the development of support networks as appropriate

If you

• Feel passionate about the needs and requirements of Carers
• Have personal or work/voluntary experience of the needs of carers
• Have knowledge of the issues surrounding people with mental health problems and the effect these can have on people’s day to day lives
