Our purpose

Within a community and primary care setting, we use a social prescribing model to reduce unnecessary appointments and help people find appropriate community and non-clinical solutions to their needs. Within a hospital setting, we work with clinical staff to ensure that people can leave hospital at the right time and with the support and care they need.

The outcome we want to achieve is to improve the health and wellbeing of people who might otherwise use statutory services, maintain their financial stability and in turn manage down demand on statutory services. We want to reduce the number of individuals entering crisis and particularly to reduce unnecessary hospital admissions.

The role

As a Senior Navigator you will work with the Programme and Area Managers to establish an effective local service for Hospital Discharge and Community Navigation including social prescribing. Leading a team of navigators, you will establish effective partnerships with the local voluntary sector, NHS providers and adult care services. Ensure that the service puts people first, empowers people to take appropriate decisions about their care, and to access support and services within their own community.

You will lead your locality team to:

  • Be the local “eyes and ears” of the integrated service

  • Deliver a high quality responsive service

  • Make sure that cases are properly and consistently reviewed and recorded

  • Contribute to the development and achievement of targets and standards

  • Provide appropriate and timely support to clients, carers or family members.

Working across both hospital and community settings, you and your team will also help prevent the need for people to be admitted to hospital, reduce their over reliance on GPs and/or home care providers, reduce their social isolation and improve their health and wellbeing.

The job

You will be employed by British Red Cross, one of the seven voluntary organisations involved in the partnership, on a fixed term contract until the end of March 2019. You will be based at the British Red Cross Offices in Hertford, but travel throughout East Hertfordshire, and especially throughout the Upper Lea Valley & Stort Valley & Villages locality will be required as part of this role.  

We need to recruit locality teams that can work flexibly as required by their communities: so successful applicants will be expected to work evenings, up to 9pm and one weekend every three weeks, all on a rota basis with existing colleagues.
